Assisted Living – Cooper Place

Philosophy

The Society’s philosophy of Assisted Living is to promote aging in place, Resident independence, dignity and self-direction.

The services provided include a private room, meals, personal services (see below), recreation and social activities, housekeeping and laundry services for the Residents. Cooper Place provides an environment that promotes and supports the independence of our Residents.

Facility

Cooper Place is a five story building that has been converted to Enhanced Assisted living. It is located in the heart of Vancouver’s downtown east-side at the corner of East Cordova and Gore Streets. There are 69 rooms – all of them have a private washroom and 15 of the rooms are wheelchair accessible. The rooms are furnished with a bed, bedside table, a chest of drawers, desk, and a chair. Residents may bring in small personal items such as pictures, television, or other appropriate furnishings.

A nurse call system is in each room to enable Residents to access assistance from staff. The facility has a no-smoking policy in all rooms and common areas. Smoking is restricted to a room on the main floor and the outdoor patios on floors 2, 3 and 4. Smoking is not allowed on the main floor patio. Alcohol consumption is only allowed in Resident rooms and not in any of the common areas.

Program of Services

Personal Services – Personal support and assistance with activities of daily living is available with:

– Bathing
– Dressing (limited assistance)
– Personal Care and Grooming (limited assistance)
– Medication reminders, management and administration
– Outside Medical appointment support and follow up

Recreation/Entertainment

Staff will arrange for socialization and entrainment that tenants can access. Programs will reflect tenants’ needs, abilities, and interests.

Meals

Cooper Place provides all meals and some snacks every day. An open and friendly dining room is available where meals are served in a comfortable and social setting. Personal preferences are considered and a dietitian is available to ensure that special needs are identified and met.

Family members, friends and loved ones can purchase meal tickets and are welcome to join residents at lunch or supper. The menu includes a variety of home cooked meals and desserts.

Housekeeping/Laundry/Linens

Daily housekeeping is provided. Bed linens and towels are provided by the facility. All items including personal clothing will be laundered by staff.

Admission

ADMISSION TO ASSISTED LIVING SERVICE

Admission to Cooper Place is based on an assessment of each proposed Resident’s individual needs. These needs are determined through an eligibility assessment conducted by a community case manager. These assessments are then reviewed by the Vancouver Coastal Health (VCH) Priority Access team.

Appropriate matches to Cooper Place are determined and made as beds become available. All inquiries regarding eligibility should be directed to your local community Health Unit. They will then arrange for a case manager to conduct an eligibility assessment as required.

A pre-admission interview with the prospective Resident and family as well as the VCH Assisted Living Case Manager and a Cooper care staff member is arranged to determine final suitability.

Fees

Residents pay user fee based on his or hers previous year’s Notice of Assessment from Revenue Canada. This rate is up to 80% of their net income and is based on the guidelines set out by the Ministry of Health. These fees cover rent, meals and most other services.

However, there are some charges the Resident is responsible for themselves. A list of these will be provided to the Resident on admission to Cooper Place.
